Not only was the wrong vitamin given, the substance in the vial was not mixed prior to being drawn up in a syringe, the wrong syringe and the wrong dose. Those are 3 major concerns I would not take as lightly as you seem to be. When something of this nature comes to your attention, I, as the patient, should not have to work as hard as I did to convince you how unsafe and unacceptable this incident was. Vitamin or drug, you should be held to the CDC rules and regulations. You are utilizing a multi-dose vial, yet have it stored in a patient area. In addition to that, your staff draws up the concentration while standing right next to the patient, increasing the odds of contamination.
I am not sure the CDC would find this an acceptable practice. I know for a fact it is not acceptable to NOT alcohol off the top of the vial before accessing it, additionally having staff not wear gloves when handling the vials and treating patients. I also do not recall seeing a sink for employees to wash their hands. Alcohol based hand sanitizer is fine once in a while, but can not be the employees only option for clean hands. You are opening the door to contamination potentially harming your patients. Since you seem to have no knowledge of safe practice, I also feel fairly confident that your vials are not dated and discarded if not used within the CDC guidelines.
When I purchased this plan, I expected to be treated by fully trained and competent staff; it is your job to make sure your staff is knowledgeable and safe with my health. What infuriates me even more, is when an issue was brought to your attention, your initial response was I should just be happier I got the more expensive vitamin. That kind of response is not only unprofessional and uncaring, but 100% unacceptable. THIS PLACE WILL CAUSE YOU MORE HARM THAN GOOD.
