improper/absent employee training (staff had to restrain a pt with no CPI training)
*staff fell hours behind on 15 minute checks due to understaffing with management refusing to assist.
*required to do CNA work without any training or certifications
*sheryl Boyd (head nurse/ manager) consistently stated she wishes the SI patients would have completed their suicide attempt.

*molly suspiciously only accepting valley view patients. (The further hospital)
*molly and other staff “fishing” for patients, resulting in us caring for patients that we are not qualified to care for. Nor was it the appropriate place for them.
*patients staying an excessive amount of time there unnecessarily. Months long
*refused to move an elderly patient to a more suitable facility and kept her for 6 months
*staff was told to to falsify information for the benefit of the company
*water leaks from the walls (never fixed)
*multiple patients complained of mold smell in their rooms because of the leaking toilets.

*overwhelming sewer smell coming from sink that pts use in the day room where they eat (never fixed)
*pts served moldy or expired food products
*locks on drawers are broken and said they won’t be fixed. (Pts have access)
