Subject ASAP Program Background: My wife, Barbara, entered Stony Brook University Hospital in Stony Brook, NY on January 24, 2018. She entered the hospital in relatively good health with the exception of needing two heart valve repair/replacement. There were complications that caused her to spend 55 days in ICU with various different problems. I had spent about 600 hours during the 55 days at her bedside advocating for return to health. Kindred in Wayne was highly recommend for her vent weaning and other aspects of her recovery. I, being her husband and her health proxy, visited the Wayne facility and was impressed so much that she was transferred that very day. I was led to believe that there would be a Family conference to go over the plan for her recovery. This meeting took place six days after she arrived. (I had moved in to a hotel nearby to be with her on a daily basis as she has no way of communication due to venting and perhaps a brain abnormality)

The meeting was missing the wound care person as well as the physical therapist. The case worker was there as well as vent therapist and the nurse on duty. I was expecting a full plan that Kindred had for her recovery, but only was given the opportunity to ask questions and they were indeed answered. But there was no plan laid out as I expected. My wife was put on a ventilator and I assume tested for various different other things. It seems that her recovery is occurring on a "let’s see basis”. I missed the Doctor the first day for some reason or another but did speak to him by phone the day after. There were unknown other Doctors assigned to her, none of which I met until days later. Because my wife was and still is incapable of moving her arms, hands and legs, I had suggested in ICU a CPM machine for her legs. This did produce movement and stimulation avoiding blood clots and giving her some movement and stretching via a robotic machine. Issues: I requested this machine as well as offering to pay the cost. After having to stress the importance of movement and stimulation to all concerned, it finally came in today. I feel that I actually had to fight hard for something that would not adversely affect my wife or the staff. A special doctor had to come in to assess the situation that caused excessive delay than was necessary.When the CPM arrived today after 8 days of begging, the cord was not long enough to use. I waited hours to no avail. I then went out to a store and purchased a 9 ft extension cord, but when I returned, there were no personnel to install it.

On this day 4/02/18 My wife’s hemoglobin was at an uncertain level of about 7 requiring an transfusion of blood. The threshold for transfusion is between 7 and 8 in most cases. The order was given somewhere around 10 AM and when I left at 7PM there was still no blood. I was ask by a nurse where my wife had had a prior transfusions due to me asking where is the blood? He replied that the people in Kindred who are in charge of transfusions were having problems with antigens and that a call was being put in to Stony Brook Hospital to solve the grace. Why did this take over 8 hours? I find myself having to ask and pry information of her BUN levels, Creatinine levels, red and white blood cell counts, co2 levels, temperature, blood sugar etc. I have learned that these are important to her well-being and recovery. Very difficult to gain information about test results and levels etc.. due to all hand written logs by the nurses. (they seem to spend 50% of their day just writing) There are times when the nurses leave for other patient and breaks (understandable) but the periods of times seem much too long. Yet, due to the fact I spend about ten hours a day at Kindred, It seems that some nurses are dedicated to one patient or another and sit outside their rooms all day long. Everything in Kindred seems to be moving in slow motion and many questions go unanswered, put off or forgot about. It is indeed a challenge. I am not looking for miracles or special treatment. I am simply dedicated to do all II can in my power to save my wife of 50 years. I had such high hopes for Kindred and the video I found about practices that are supposed to go on there. I have been at her side for 68 days. Please help me fight this battle

Today 4/04/18 It really gets bad, bordering on malpractice (going to ask my attorney) Went into my wife’s room expecting to see a catheter in her neck for kidney dialysis as prescribed by the renal doctor associated with Kindred. When I asked about it, the nurse told me the surgeon was there the night before but due to her allergy of lidocaine ( has never had an allergy to this anesthetic) Was told he was coming in today to perform procedure with a different anesthetic. When he did come into the room, I asked how long will this take. He said 10-15 minutes. I excused myself as I did not want to view my wife’s neck cut. I waited one half hour, went back and it was still going on. I left again and when I returned I asked the nurse where the catheter was? She replied he put it into her right femoral artery. When I ask why the nurse said "he was in a hurry and had to see another patient, somewhere” I was then asked to sign a consent form after the fact. I said no, I did not feel comfortable signing something after it took place. I was then told my wife would be refused dialysis unless I signed. They held the dialysis as hostage! I finally agreed to sign due to the fact I did not want to endanger her life in any way. Then I was told the paper was on a different floor and an audit was going on and the paper could or would not be produced. I asked for a new blank release and was told that was not available. The dialysis machine was then removed from the room, even after I had agreed to sign. Then it was brought back into the room. I have videos and photos of all that happened. I was then told that they would go ahead with the dialysis. Management should all be terminated for malpractice and ineffectiveness. They have no people skills whatsoever. Kindred should be ashamed of what has happen both today as well as the rest of my wife’s stay. Instead of receiving care and kindness, we received only grief. I am making arrangements to get her out of Kindred ASAP
